Planned Period of Work

Goal: Address community poverty-related needs by strengthening the community and the development of infrastructure through a community building process. The process will engage individual citizens, citizen groups, and agencies to address community needs and will enable the community to develop and implement a strategic plan resulting in sustainable activities to meet community needs.

Activity 1: Work on a plan to provide a comprehensive approach to Communicable Disease (Chlamydia in particular) management that involves all agencies involved.
Step 1: Review the history of communicable disease in Richland County and the work that was done by the student nurse.
Step 2: Participate in the Board of Health meetings.
Step 3: Meet with Julie Brodhead to work out some goals for over-lapping (provider, SHC, Migrant, etc.) communicable disease issues.
Step 4: Contact the State Immunization and Communicable Disease Bureau Chief. Work with them on their state project.
Step 5: Develop a strategy for addressing Chlamydia in Richland County.
Step 6: Present the strategy to the department administrator and other appropriate staff.
Step7: Present the strategy and results to the Board of Health.
Step 8: Build the strategy into the current Health Department Strategic Plan.
