Immunizations are provided to all age groups.

Clinic Hours

Every Monday, Tuesday and Wednesday: Noon - 4:30 pm

To accomodate everyone's schedules, we also offer clinic hours on the third Thursday of each month from 1pm to 4pm and a late clinic on the second Wednesday fom 4pm to 6pm. Please call ahead if you plan to come during our late clinic.

To help us serve you better, please bring:

• your immunization record card.
• any of the following that applies to you - Medicaid card, CHIPS card, MUST insurance number, Medicare card

You should also be ready to answer the following questions:

• Is your child enrolled in Medicaid/Chips?
• Do you have MUST insurance?
• Do you qualify for IHS (Indian Health Service) or other federally funded insurance?
• Do you have health insurance that pays for vaccines?
